Consumer Direct Payment

Dr. Eads is happy to see you even if you have insurance that she does not contract with.   Some insurances have                             'out of network benefits.'  These are benefits that help cover the cost of you seeing Dr. Eads for an appointment.                                For example, a typical benefit is 80% coverage by insurance, and 20% paid by you.  You should check with your insurance to      see how much, if any, such benefit you may have.  You will be responsible for the entire cost of the office visit, and must pay at the time of service.  At your request, Dr. Eads will complete a standard paper claim (HCFA 1500) for that visit for you to mail to your insurance company and they will mail you a check for the amount they cover.  Other services such as labs, x-rays, etc will be covered as usual, including prescriptions.  Only the services rendered by Dr. Eads fall into the 'out of network' category, as long as you go to 'in network' facilities for your labs, etc.

 

Services provided by Dr. Eads that will be considered 'out of network' include:       

Office Appointments or Visits

Phone Appointments or Visits

Virtual Visits

Authorizing Prescription Refills

Completing Forms


  • Payment Structure

    As of January 1, 2008, Dr. Eads no longer has contracts with insurance.  She is able to see anyone, including those without insurance, and can prepare a completed claim form for you to submit to your insurance company if you have 'out of network' benefits.  For those with Medicare, Dr. Eads has 'opted out' of Medicare, which means you will not be able to submit your costs associated with a service done by Dr. Eads for reimbursement from Medicare.  Prescriptions, labs, imaging studies, and specialist visits should be covered as usual by your insurance.
