There are four different membership levels to suit your particular needs.  The Silver Membership is designed for those who rarely need to work with a physician.  For example, they may only need their once a year sinus infection treated and an annual exam.  The Gold and Platinum Memberships are for those that have issues that threaten their health or require more invention, such as diabetes, high blood pressure, or depression.  These folks need more of the doctor's time to get and keep their illness under control.  The Executive Membership is designed for those that need to minimize wasted time by having the doctor come to them, or otherwise have difficulty making it to the doctor's office, or just prefer the convenience.

Consumer Direct Payment
  • Silver Membership
    This membership level is intended for those who are healthy and free from long-term problems such as high blood pressure, high cholesterol, diabetes, and depression.  For example, a typical person who fits this category will have two doctor visits a year; one for a sinus or bladder infection, and one for their annual physical and exam.  An Annual Access Fee is assessed, and  services are paid for as they are used.
  • Gold Membership This membership level is intended for those who have long-term conditions that require management (such as high blood pressure, diabetes, high cholesterol, and depression) or who want predictable monthly costs.  For example, a typical person for this category may have several longer doctor appointments throughout the year in addition to their annual physical and exam.  Most people in this membership level are over 34 years old.
  • Platinum Membership This membership level is intended for those who have serious or multiple long-term conditions requiring more intense management (such as uncontrolled high blood pressure, diabetes or high cholesterol; rheumatoid arthritis, cancer, etc.) that requires the care of a specialist, or anticipate hospitalization.  It adds to the services included in the Gold Membership.  Dr. Eads will accompany you on three specialist visits or communicate with the hospital specialist three days per year.  For example, a typical person for this category needs many longer doctor appointments throughout the year with their primary doctor and specialist in addition to their annual physical exam.  Most people in this membership level are over 39 years old.
  • Executive Membership (click for more information)
    This membership level is intended for those who have tight time constraints or desire the convenience of their doctor coming to them.  This category builds on the Gold Membership by adding home visits and place of business appointments.  For example, a typical person for this membership needs very efficient use of their time or have difficulty travelling to the doctor’s office.  Most people in this membership level are over 39 years old and may travel or stay mainly at home.
